Office Policies

Thank you for choosing Riverside Family Dental as your primary dental care provider.

We are committed to providing you with high quality and affordable dental care.

Please review our policies listed below, as they are important to understanding the services offered at our office as well as how your insurance claims are filed and how your payments are processed.

Registration

During your first appointment you will be asked to complete forms which will ask for information such as your name, address, social security number, insurance information, and health history as well as any medications you are taking. If possible, please bring this information with you. Please note, these forms are updated every 3 years.

 

Proof of Insurance

Our patient information forms will need to be completed before seeing the doctor. We must obtain a copy of your driver’s license, current valid insurance card/paperwork, and social security number in order for us to be able to file insurance on your behalf. We collect all three of these in order to avoid issues with insurance companies while filing claims on all of our patient's behalf. If you are unable to provide us with the correct insurance information in a timely manner, your insurance may deny coverage for your treatment(s) and you may become fully responsible for the balance of your claim(s). If you are unable to provide us with your SSN, you will be considered a self-pay patient and will be responsible for filing your own claims to your own insurance provider (our office will provide you with the form to file yourself).  Our office is very experienced in dealing with numerous insurance providers and we will try our best to assist you in any way possible.

Appointments

To provide the best possible care, we prefer to see our patients on an appointment basis. Please call us in advance so that a special time can be reserved for you. If you have an emergency, we will do our best to accommodate you and address your urgent treatment needs. Our goal is to keep our patients healthy and smiling. If you are unable to keep an appointment, please reschedule or cancel your appointment within 24 hours so that we might be able to accommodate other patients and their treatment needs.  

New Patient Disclosure

Riverside Family Dental has a same-day payment policy for all NEW patients at our office. All NEW patients will be asked to pay the full amount of their charges, even if filing insurance, at the time of their appointment. We accept cash, checks, Visa, MasterCard, Discover, and Care Credit. If you are a new patient who would like our office to file dental insurance on your behalf, our insurance coordinator will submit claims to your insurance company for all services you receive. Claims typically take 3-4 weeks before our office hears back from the insurance company. However, depending on the insurance carrier and procedures that were submitted, it can take longer for the insurance to process your dental claim. If your insurance pays and this results in a credit on your account, you may request a refund for the credit amount. Riverside Family Dental will provide a refund to patients after payment from their insurance company has been fully processed by our office. Once you are an established patient with our office and we are familiar with your specific insurance policy and coverage, our office will only ask you to pay your expected out of pocket costs (what the insurance does not cover) based on your plans coverage. 

Insurance Claim Submissions

Our Office is committed to helping you maximize your insurance benefits. Since insurance policies vary, we can only estimate your coverage and cannot guarantee coverage due to the complexities of insurance contracts until after your claim has fully been processed by the insurance carrier. Our office is able to accept/file most insurance plans on your behalf, even if we are not an in-network provider.  While we are only in-network with Delta Dental plans, we will still gladly file insurance on your behalf and our team will do our best to maximize your insurance coverage to help decrease your out of pocket costs. If you have any questions about your specific insurance coverage or claims, please do not hesitate to contact the office, we are always here to help our patients.

Payment

Our office accept payments with cash, checks, Visa, MasterCard, Discover, and Care Credit. We do not want your payment option to prevent you from getting a beautiful smile. That is why we are proud to accept payment from all major credit cards. Additionally, our office offers different payment plan options to help make your treatment more affordable. We do not want finances getting in the way of taking care of your health or your smile.

 

Questions About Your Billing Statement

If you have any questions about your billing statement, we encourage you to contact our office and speak with our Insurance Coordinator. Our Insurance Coordinator will review the EOB’s (explanation of benefits) from the insurance company and help answer any questions. If it is found that an error was made by Riverside Family Dental in relation to your billing statement, it will be corrected immediately. Insurance can be complicated, so please do not hesitate to ask us questions. We strive to make sure our patients are taken care of and we are always happy to help.  

 

Patient Refund

If your insurance pays more than what was originally anticipated and this results in a credit on the account, you may request a refund for the credit amount. Riverside Family Dental will provide a refund to patients after payment from their insurance company has been fully processed by our office.

Insurance Disclaimer

Verification of insurance coverage and/or benefit information is not a guarantee of payment. Benefits will be determined once the member's insurance processes the claim(s) and will be based upon, among other things, the member’s coverage.

 

Returned Check Policy

Any check returned to our office as insufficient funds shall be charged a $30 service fee in addition to the value of the check.
